Fit Testing

SVHC has Qualitative Fit Test Kits available for loan for a period of time not to exceed 5 business days, (inclusive of dates received and returned.) Should you require additional time, please submit another Loan Request Form, PRIOR to your initial agreed-upon return due date. For valid reasons, an extension may be granted, providing another facility/agency has not already requested use of your assigned kit/kits. SVHC will make every effort to grant you additional time, based on regional priority needs. Every effort should be made to return fit test kit(s) by close of business on the 5th day of the agreed-upon loan period to keep this asset available for deployment if needed.

To borrow this asset, your facility must meet the following conditions:

• Have a current MOU or Letter of Support (LOS) on file with SVHC
• Have a confirmed date for your fit test event
• Have a trained Fit-Tester available to administer the tests
• Have a Respiratory Protection Program in place & will maintain records of all fit-testing documentation required by OSHA, CMS, VDH, and internal policies
• Make arrangements to Pick up/Return the Fit Test Kit(s) with ample notice to SVHC
• Agree to all Fit Test Kit Loan Program terms

• two sets replacement nebulizer inserts
• one collar assembly
• sensitivity solution
• fit test solution
• laminated user guide
• laminated step by step instructional sheet
